Okay, it doesn't get any easier or more spelled out than this.
Be careful on the parts where you walk up to the D chord.  It's
kinda misleading when you listen to the song, because the chord
voicing is going up, but Hanks voice is going down.  It's actually
a kind of neat thing about the song.  If you don't have a really low 
voice like Hank, you might have to adjust the way you sing it.  
Don't forget to drop your low E string to D.
