Hasn't been tabbed yet, so I listened to it and figured it out.
Pretty dang easy.

One thing:
Didn't feel like taking the time to figure out the intro part.
So either figure it out yourself or pick the chords on your clean channel.

Not sure if they do this live.
I can hear a guitar faintly in the background play this during the chorus:

Main riff again. Then verse again.

However, during the second half of the verse, gtr 1 doesn't do the same strum and hold 
the D chord (577).
During this, you do a distorted palm-mute of that chord (listen to hear the pattern) 
you get to the "changing for the worse" part.

Gtr 1 does this palm-muted part too during the second half of the verse.
During the first half, it doesn't play anything (cept for some feedback).

Pre-chorus again, then chorus again.

Then quickly slide down to nowhere and mute the strings for 7 more bars.

P.S. Not sure which guitarist plays which parts.
Might have gotten some mixed up, but they're all there, and I'm pretty sure they are correct.
Not 100%, but they sound near perfect to me.
